This form is for Assembly 2026 student session planning. Parents may submit multiple children, and older students or college-age participants may register themselves.

One parent entry Parents or guardians complete contact information once, then add each child below.

Self registration If you are not a minor, choose self registration and enter your own contact details and selections.

Elementary attendance For Children's Church, please choose Saturday, Sunday, or Both so the team can plan for each service.

I acknowledge and agree to the liability release for my child to participate in Assembly student activities. I consent to the general photo release for Church of God of the Union Assembly, Inc. Assembly activities.

Photo Release for Minors: I do consent and allow my child to be filmed, videotaped, and/or photographed for use by the Church of God of the Union Assembly, Inc. I understand that the images may be used in print publications, online publications, presentations, websites, and social media.
